Job summary

Residential Home Health and Residential Hospice is seeking a Home Health RN to provide skilled nursing care in patients' homes across the Ann Arbor area, including Dexter, Chelsea, Ypsilanti and surrounding towns. This full-time role includes a $10,000 first-year bonus and opportunities for ongoing rewards as you grow with our nationally recognized leader in home health and hospice.

You will deliver patient-centered care, conduct assessments, manage medications, and educate families on treatment

Qualifications

• Active RN license in the state you work
• Minimum 1 year of RN experience delivering skilled nursing care
• Valid driver’s license required for field visits

Responsibilities

• Deliver skilled, patient-centered care in a home health setting in collaboration with an interdisciplinary team
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans
• Maintain compliant EMR documentation and accurate charting
• Work independently while staying connected with the team
• Travel to patient homes within assigned territory

RN Qualifications

• Active Registered Nurse, RN license in the state you work
• One year of RN experience and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
