Ambercare Hospice is seeking a compassionate and dedicated Registered Nurse On Call to join our newly restructured team in Silver City, NM. Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR First Aid Certification.
• Must be knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Assume primary responsibility for patient and family caseload including assessment, eligibility, admission, processing, planning, implementation and evaluation phases of the nursing process.
• Obtain data and assess information from patient’s physical, psychological, social and spiritual factors that may impact needs and initiate intervention by the interdisciplinary team.
• Initiate communication with the attending physician and interdisciplinary team for coordinating optimal care.
• Provide regular visits to reassess the patient’s needs and update the plan of care.
• Perform clinical pronouncement of death and provide support to the family.
• Maintain accurate patient records, charts, progress notes, reports and timely care plans.
• Supervise and document delivery of care by Home Health Aide as required by guidelines.
