Job summary

UnitedHealth Group in the United States is seeking an RN On-Call Hospice to provide after-hours clinical support, triage patient needs, and coordinate care to ensure continuity and quality outcomes.

This role delivers remote and field-based nursing support, assesses patient conditions by phone or in-person visits, and collaborates with physicians, care teams, and caregivers to deliver timely, compassionate care.

Qualifications

• Current RN license for the assigned state(s).
• Valid CPR certification.
• 1+ years of nursing experience, preferably in hospice, home health, or acute care.

Responsibilities

• Provide on-call nursing support, triaging patient needs and determining interventions.
• Assess patient conditions via phone and in-person visits and upscale care as needed.
• Coordinate with physicians, care teams, and caregivers to ensure timely, effective care delivery.
• Deliver patient and caregiver education related to symptom management and care plans.
• Maintain accurate and timely documentation of all patient interactions and clinical decisions.
