UnitedHealth Group in the United States is hiring a Registered Nurse - Hospice On-Call (After Hour). This role focuses on remote and field-based nursing, triaging patient needs, and coordinating care for continuity and quality outcomes on an on-call schedule (Friday 5pm - Monday 8am).

You will assess patients by phone or visit, educate patients and caregivers, and document clinical decisions. Requires RN license in assigned states, CPR, and 1+ year of hospice/home health or acute care

Qualificações

• Current RN license valid for the assigned state(s).
• Valid driver’s license with reliable transportation and liability insurance.
• Current CPR certification.
• 1+ year of nursing experience, preferably in hospice, home health, or acute care.

Responsabilidades

• Provide on-call nursing support, triaging patient needs and determining appropriate interventions.
• Assess patient conditions via phone and/or in-person visits and escalate care as needed.
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery.
• Deliver patient and caregiver education related to symptom management and care plans.
• Maintain accurate and timely documentation of all patient interactions and clinical decisions.
