Job summary

Chapters Health System in Aldie, VA seeks a Per Diem Registered Nurse to provide compassionate palliative and supportive care within the Hospice House team. You will assess patient needs, coordinate care with the IDG, and utilize the nursing process to ease symptoms and improve comfort.

You’ll work with an interdisciplinary team across facilities, document care in the EMR, educate families, and participate in discharge planning and bereavement support.

Qualifications

• Current RN license in the state where employed.
• Active BLS for healthcare professionals.
• Minimum 1 year inpatient nursing or hospice experience for external candidates; internal: 6 months LPN in hospice/home health.
• Experience with EMR/EHR systems.

Responsibilities

• Provides and manages direct care to patients and families as part of IDG.
• Coordinates Plan of Care reflecting patient evolving needs.
• Educates patient and family about disease, palliation, safety practices.
• Reports changes in condition to IDG or other health professionals.
• Attends patient deaths and prepares body for transport; manages medications.
