Job summary

Chapters Health System seeks a Per Diem Registered Nurse to provide compassionate palliative and supportive care within the Hospice House setting. You will assess patient needs, coordinate the Plan of Care with the Interdisciplinary Group, and support patients and families with comfort and dignity.

Candidates must hold an active RN license in the state, current BLS, and be adept with EMR/EHR systems. A valid driver’s license and reliable transportation are required; strong English communication

Qualifications

• Current RN license in the state where the employee will be working (Florida).
• Active BLS for healthcare professionals from AHA or Red Cross.
• Minimum 1 year in-patient nursing or hospice experience for external candidates; 6 months LPN with hospice/home health for internal applicants.
• Experience with EMR/EHR systems.

Responsibilities

• Provides and manages direct care to patients and families as part of the IDG.
• Coordinates the Plan of Care to reflect evolving patient needs.
• Educates patient and family about disease, palliative interventions, safety and care.
• Reports changes in patient condition to IDG or other health professionals.
• Participates in evaluating hospice referrals/admissions for level of care.
