Saint Agnes Home Health and Hospice in California is seeking a Hospice Registered Nurse (RN) to cover on-call time for Friday and Saturday nights, with part-time hours and night shift. You will assess patients, develop care plans, and deliver compassionate, patient-centered home care under Trinity Health At Home guidelines.

The role requires an active RN license in California, at least 1 year of nursing experience (home health preferred), and reliable transportation.

Qualifications

Current RN license in California.
Minimum 1 year of nursing experience; home health experience preferred.
Home healthcare experience preferred.
Valid driver’s license and reliable transportation.
Strong computer skills and a commitment to ethical, mission-driven care.

Responsabilités

Provide skilled nursing care in accordance with physician orders and organizational policies.
Perform comprehensive assessments and develop individualized care plans.
Collaborate with physicians, therapists, social workers, and home health aides.
Supervise and support LPNs and Home Health Aides.
Ensure compliance with regulatory standards and contribute to quality improvement initiatives.
Promote patient satisfaction and positive health outcomes.
