Job summary

Trinity Health At Home, Saint Agnes Home Health and Hospice in California, seeks a Hospice Admission RN for an afternoon 2pm–11pm shift. You will perform comprehensive assessments, develop personalized care plans, and initiate the hospice plan of care with clear communication.

You bring an active California RN license, at least 1 year of nursing and home health experience, and strong computer skills to deliver compassionate, compliant care.

• Current RN license (California)
• Minimum 1 year of nursing experience
• Home health care experience preferred
• Valid driver’s license and reliable transportation
• Strong computer skills and a commitment to ethical, mission‑driven care

Responsibilities

• Provide skilled nursing care in accordance with physician orders and organizational policies
• Perform comprehensive assessments and develop individualized care plans
• Collaborate with physicians, therapists, social workers, and home health aides
• Supervise and support LPNs and Home Health Aides
• Ensure compliance with regulatory standards and contribute to quality improvement initiatives
• Promote patient satisfaction and positive health outcomes

Physical & Mental Requirements
