Job summary

Trinity Health At Home's Saint Agnes Home Health and Hospice in California is seeking a Hospice Registered Nurse (RN) for the 2pm-11pm afternoon shift. You will assess patients, develop care plans, and deliver compassionate at-home hospice care with an emphasis on comfort and dignity.

The role requires California RN licensure (BSN preferred), at least 1 year of nursing experience, and a drive to provide high-quality care. Sign-on bonus of $10,000 and comprehensive benefits are available.

Qualifications

• California RN license is required; minimum 1 year of nursing experience.
• Home health care experience is preferred.
• Strong computer skills and reliable transportation are required.

Responsibilities

• Provide skilled nursing care per physician orders and policies.
• Perform comprehensive assessments and develop patient-specific care plans.
• Collaborate with physicians, therapists, social workers, and home health aides.
• Supervise and support LPNs and Home Health Aides.
• Ensure regulatory compliance and contribute to quality improvement.
• Promote patient satisfaction and positive health outcomes.
