Job summary

Mercy San Juan Medical Center is seeking a Night On Call Per Diem Hospice Registered Nurse (RN) to provide compassionate care and support in patients' homes, enabling families to care for their loved ones at home. The on-call shift runs 1700–0830, with collaboration across a broad care team.

Requirements include CA RN license and BLS; drivers with CA license and auto insurance are needed for after-hours admissions and on-call needs.

Qualifications

• Minimum one (1) year hospice nursing experience in the past five years.
• Active RN license in California.
• BLS certification - American Heart Association.
• Current California Driver's License, Auto Insurance and access to an operating vehicle.

Responsibilities

• Deliver direct patient care in the home hospice setting.
• Coordinate with physicians, nurses, social workers, counselors, chaplains, home health aides and volunteers.
• Meet productivity standards and document per guidelines.
• Ensure regulatory compliance and quality of care.
• Communicate across disciplines and maintain care plans.
