SCAH-Sutter Care at Home - Valley in Sacramento seeks an experienced RN to join our hospice team. You will assess, coordinate, and deliver holistic nursing care, educate patients and families, and collaborate with physicians and other providers to ensure high-quality care in line with hospice philosophy and policies.

Requirements include CA RN license and BLS, with 2 years of hospice or similar experience. This part-time night role offers 24 weekly hours, union support, and benefits eligibility.

Qualifications

• Graduate of an accredited school of nursing.
• 2 years recent relevant experience.

Responsibilities

• Assess, coordinate delivery and evaluation of nursing care for hospice patients.
• Educate patients and families with holistic, individualized care plans.
• Collaborate with physicians and healthcare providers to ensure high-quality care.
• Maintain patient privacy and adhere to HIPAA and regulations.
