Job summary

First Call Hospice in Citrus Heights, CA is seeking a part-time Registered Nurse to provide home-based hospice care. The RN will assess patients, develop care plans, and coordinate services with families in the community.

The successful candidate will hold a California RN license, preferably have hospice experience, and be able to perform comprehensive nursing duties including medication administration and patient education.

Qualifications

• Active California RN license required.
• Hospice experience preferred.
• Ability to perform patient assessments and develop care plans.

Responsibilities

• Assess patients and develop care plans based on nursing diagnosis.
• Provide professional nursing care and administer prescribed medications.
• Coordinate patient care as Case Manager and educate patients and families.
• Assist patients with activities of daily living and support self-sufficiency.
