SCAH-Sutter Care at Home - Bay is seeking an experienced Registered Nurse to provide skilled palliative and end-of-life care in the community, coordinating with an interdisciplinary team and ensuring comprehensive medical management of hospice services for patients and their families.

Requires nursing licensure in California, BLS certification, a valid driver's license, and the ability to work with a collaborative care team in home settings. Weekend shifts and full-time schedule are typical.

Formación

• Graduate of an accredited School of Nursing.
• RN-California license.
• BLS Healthcare Provider certification.
• Driver's license with automobile insurance.

Responsabilidades

• Provide skilled palliative and end-of-life care in the community setting.
• Coordinate and manage hospice services with an interdisciplinary team.
• Develop and implement an interdisciplinary care plan for patients and families.
