Job summary

SCAH-Sutter Care at Home - Valley is seeking a Registered Nurse with hospice experience to support patient care and family education in California. You will assess and coordinate nursing care, delivering direct patient care and leading an interdisciplinary plan of care.

We value collaboration with physicians, family support, and adherence to policies to ensure a high-quality patient experience. Evening shifts, full-time schedule, and 40 hours per week are typical.

Qualifications

• Graduate of an accredited school of nursing.
• Ability to perform structured nursing assignments in hospice care.
• Competence in coordination and education of patients and families.

Responsibilities

• Assess, coordinate, deliver and evaluate nursing care for Hospice patients.
• Provide direct patient care and patient/family education.
• Develop and manage an interdisciplinary plan of care.
• Collaborate with physicians and other providers; maintain timely communications.
• Uphold privacy and regulatory standards; ensure quality patient experience.
