Job summary

Constellation Health Services is seeking a Hospice RN Clinical Liaison to serve the Philadelphia area. The role requires an active RN license and experience in home health care, with a focus on expanding partnerships with hospitals, SNFs, ALFs, physician offices and community organizations.

You will assess patients and families, develop care plans, coordinate care as case manager, and generate Medicare referrals while building lasting relationships with social workers and healthcare partners.

Qualifications

• Graduate of an accredited school of nursing.
• One to two years of acute care experience.
• Current licensure in state and CPR certification.
• Bachelor’s degree; one year home health care experience preferred.

Responsibilities

• Completes initial patient and family assessment and determines home care needs.
• Develops and revises care plans based on nursing diagnosis and patient status.
• Administers medications and treatments as prescribed.
• Acts as Case Manager for assigned caseload and coordinates patient care.
• Communicates with physicians and community partners to coordinate care.
• Participates in on-call duties as defined by policy.
