Job summary

Constellation Health Services seeks a compassionate Hospice Registered Nurse Case Manager to serve patients in their homes, assisted living facilities, and skilled nursing facilities throughout the Greater Philadelphia area and surrounding counties.

You will perform skilled nursing care, coordinate the care plan, and collaborate with an interdisciplinary team to ensure high-quality patient outcomes. Weekend/holiday coverage may be required, with a supportive, family-centered culture.

Qualifications

• Graduate of an accredited RN program.
• Registered Nurse license in the state of employment.
• 1–2 years of RN experience; hospice care strongly desired.
• Basic knowledge of iOS operating system.

Responsibilities

• Provides skilled nursing care in home/hospice settings including medications and treatments.
• Performs assessments and coordinates the plan of care with disciplines and family/caregivers.
• Observes and reports changes to nursing supervisor and physician.
• Participates in weekly Interdisciplinary Team Meetings with status updates.
• Maintains documentation per agency standards using the computer system.
• Weekend and holiday coverage as needed.
