Job summary

Constellation Health is seeking an experienced RN Clinical Liaison to join our team in Cincinnati, OH. The RN will plan, organize and direct home health services, and engage community partners to expand Hospice & Palliative Care opportunities.

Responsibilities include patient assessments, care planning, coordination with physicians and social workers, on-call duties, and building relationships with hospitals, SNFs and physician offices to generate Medicare referrals.

Qualifications

• Graduate of an accredited school of nursing.
• 1–2 years of recent acute care experience.
• Current licensure in state and CPR certification.
• Excellent oral and written communication; problem solving and math skills.
• Bachelor’s degree with one year home health care preferred.
• Willingness to drive and network with community partners.

Responsibilities

• Assess patients and families to determine home care needs and develop care plans.
• Coordinate patient care with physicians, nurses, social workers, and home health aides.
• Maintain ongoing patient and family education; update plans as status changes.
• Participate in on-call duties and arrange equipment/services.
• Develop relationships with Hospitals, SNFs, and physician offices to grow referrals.
