Job summary

Bluegrass Care Navigators seeks a dedicated Hospice RN to coordinate care for terminally ill patients and families, focusing on comfort and quality of life. The role emphasizes education of hospital staff, referrals, and seamless transitions of care across home, nursing facilities, andHospice Care Centers.

The position requires an active RN license, 2+ years nursing experience (prefer hospice/home health/ICU), CPR, and ability to travel locally.

Qualifications

• RN license in good standing with state nursing board.
• Graduate of an accredited school of professional nursing.
• Minimum of two years general nursing experience; hospice/home health/ICU/med-surg preferred.
• Pain and symptom management competency.
• Demonstrated ability to support patients and families under emotional stress.
• Clear understanding of death and dying concepts and hospice philosophy.
• Knowledge of Medicare Conditions of Participation for hospice; regulatory requirements.
• Current CPR certification.
• Strong written and verbal communication; professional standards adherence.
• Ability to travel locally and access hospitals, nursing facilities and other care settings.

Responsibilities

• Liaise between organization and hospital staff; educate on services and programs.
• Facilitate effective communication with physicians and interdisciplinary teams.
• Lead development, implementation, monitoring, and supervision of initiatives to increase referrals.
• Assist with referral coordination and ensure seamless transitions of care.
• Provide oversight and case management for hospitalized hospice patients.
• Report directly to the Executive Director.
