Job summary

Bluegrass Care Navigators is seeking an RN for PRN hospice care to support patients and families in home, nursing facilities, or hospitals. You will provide skilled nursing care as prescribed, communicate with the care team, and ensure comfort and dignity for patients at end of life.

The role requires an active RN license, CPR certification, and the ability to work with minimal supervision while collaborating with an interdisciplinary team.

Qualifications

• Graduate of an accredited school of professional nursing.
• Currently licensed as an RN through the State Board of Nursing.
• At least one year of general nursing experience preferred, hospice/home health/critical care experience a plus.
• Excellent observation, nursing judgment and written documentation skills.

Responsibilities

• Provide skilled nursing care to patient/family as prescribed by physician and in compliance with nursing regulations.
• Report to Executive Director and/or Clinical Nursing Officer.
• Coordinate care within an interdisciplinary team.
• Maintain patient comfort and ensure pain management as part of hospice philosophy.
