Job summary

Bluegrass Care Navigators seeks a Hospice RN Case Manager to deliver compassionate, comprehensive health care to patients in a home setting. You will actively listen, develop holistic care plans, and coordinate services for patients and families in Fayette and Jessamine counties.

The role requires a Kentucky RN license, CPR certification, and at least 1 year of nursing experience; driving is required to visit patients at home, nursing facilities, or hospitals. Monday–Friday, 8:00am–5:00pm.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed as a Registered Nurse in Kentucky or via a Compact State Board of Nursing.
• Minimum of 1 year nursing experience preferred; new grads considered.
• Demonstrates strong clinical nursing skills and professionalism under emotional strain.
• Provides compassionate, patient-centered care with respect and cultural sensitivity.
• Maintains CPR certification and driving eligibility with a valid automobile.

Responsibilities

• Deliver comprehensive hospice care to patients in home or patient-reported settings.
• Act as patient advocate, coordinating care for patients, families and caregivers.
• Develop, implement, and monitor a holistic plan of care based on patient goals.
• Provide symptom management through medications and non-pharmacologic interventions; monitor efficacy.
• Communicate effectively within an interdisciplinary team to ensure quality care.

As a Bluegrass Care Navigators' Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.

Position Summary

The Hospice RN Case Managerdeliverscompassionate and comprehensive health care services to Hospice patients in a home care setting,orwhereverthe patient calls home.Home care settingsmayconsist ofa private residence, skilled nursing facility, personal care home, and/or other interim livingarrangements.The Hospice RN Case Manager’sgoal for excellentpatient care standardsbegins with active listening to create and implement a holistic plan of care based on the patient’s goals andpreferences.The Hospice RN Case Manager serves as the patient advocate to support, guide, and coordinate care for patients, families, and caregivers as they navigate Hospice Care.The plan of care will include managing symptoms throughpharmacologic and non-pharmacologic interventions foroptimalcomfort, functionality, and safety. Frequent monitoring and follow-up arerequiredto evaluate the efficacy of treatments, plans of care, and to offer emotional support to both the patient and the caregiver. A Registered Nurse in this position will havestrong communicationskills, the competence to work autonomously, professionalism to work within an interdisciplinary team, and the ability to provide care with empathy. Position Qualifications

• Currently licensed as a Registered NursethroughtheKentucky Board of Nursingor through a participating CompactStateBoard of Nursing.
• Minimum of one (1) year of general nursing experience preferred; new graduate nurses will be considered.
• Demonstrates strong clinical nursing skills and the ability tomaintainprofessionalism during emotionally challenging situations.
• Provides compassionate, patient-centered care while serving diverse populations with respect and cultural sensitivity.
• Adheres to accepted professional, ethical, and legal standards of nursing practice.
• Demonstrates understanding of and commitment to hospice philosophy and the principles of death and dying.
• Maintains current CPR certification.
• Must be a licensed driver with an automobile in good working order, that isinsuredin accordance withstateminimumrequirements.
