Job summary

Heartland Hospice is seeking an experienced Hospice Clinical Liaison to streamline intake, coordinate admissions, and build strong relationships with hospitals, discharge planners, physicians, and families. You will educate patients on hospice care, help with eligibility determinations, and support community outreach.

Ideal candidates hold an RN license with 3–5 years of hospice experience, BSN preferred, and CPR certification.

Qualifications

• Licensed RN with hospice experience.
• BSN preferred with hospice background.
• CPR certification required.

Responsibilities

• Receive and respond to referrals from hospitals, home health, physicians, case managers, SNFs and other healthcare professionals.
• Assess patient eligibility for hospice by reviewing records, clinical evaluation, and communications with physicians and families.
• Educate patients and families on hospice care, processes, and benefits with clarity and compassion.
• Coordinate intake and admission, ensuring all legal, clinical, and payer requirements are met.
• Serve as liaison between referral sources and hospice team, ensuring seamless handoffs and rapid response.
