Job summary

Gentiva in Alabama seeks an experienced Hospice Clinical Liaison to streamline intake, referrals, and admissions while building strong relationships with hospitals, physicians, and case managers.

The role requires RN licensure with hospice experience (3–5 years), plus excellent assessment, documentation and education skills. Travel to hospitals and facilities is expected, with a compassionate, team-focused approach.

Qualifications

• Licensed nurse with hospice experience required (3–5 years).
• BSN with hospice experience preferred.
• Strong assessment, documentation and communication skills.
• Ability to educate patients and families about hospice services.

Responsibilities

• Receive and respond to referrals from hospitals, home health and physicians.
• Assess patient eligibility and coordinate intake/admission with legal and payer requirements.
• Educate patients and families on hospice care and what it includes.
• Serve as a liaison between referral sources and hospice care team for smooth handoffs.
• Make daily visits to hospitals and facilities and build referral partner relationships.
