Job summary

Lumina Hospice & Palliative Care is seeking a Primary Nurse to serve assigned patients and ensure comprehensive care by the hospice interdisciplinary team. You will coordinate and deliver nursing services with a patient‑centered approach, ensuring timely documentation and regulation compliance.

Ideal candidates bring at least 3 years in hospice or related settings, Oregon RN licensure, strong communication skills, and a willingness to travel to patient homes with a reliable vehicle.

Qualifications

• Oregon RN license is required and must be current and unencumbered.
• Minimum 3 years of professional nursing in hospice, home care, ICU, or acute care; hospice experience preferred.
• Ability to work effectively with patients, families and interdisciplinary colleagues.
• Must have a valid driver’s license and access to a reliable automobile for home visits.
• Proficient in email, word processing, internet, keyboarding, and capable of learning medical software for charting.

Responsibilities

• Assess, evaluate, and reevaluate patient and family needs; organize resources.
• Coordinate care as case manager across hospice disciplines.
• Provide nursing care addressing physical, emotional, and spiritual needs.
• Counsel patients and families on symptom control, medications, and care options.
• Assist with patient admissions, educate families, arrange services and medications.
• Collaborate with physicians, pharmacies, and care facilities to ensure optimal care.
• Communicate effectively with Lumina staff and participate in care planning with IDG members.
