Job summary

Presbyterian Healthcare Services is seeking an experienced RN for hospice care in the United States. The role focuses on compassionate, end-of-life care with interdisciplinary collaboration and leadership responsibilities.

The position requires an Associate Degree in Nursing, a compact/multi-state RN license, and at least one year of direct patient care or home health experience, plus a valid driver’s license and BLS. On-site role with competitive compensation.

Qualifications

• BLS certification required at time of hire.
• One year minimum in direct patient care or home health care.
• Valid driver’s license is required.
• Registered Nurse licensure must be compact/multi-state.
• Associate Degree in Nursing is required.

Responsibilities

• Deliver comprehensive end-of-life care with clinical assessments and care plans.
• Administer physician orders and monitor symptom management.
• Lead interdisciplinary care coordination with nurses, CNAs, physicians, and partners.
• Precept and mentor new hires and clinical students.
• Maintain accurate clinical documentation in line with regulations (state, federal, QAPI/PIP).
• Manage caseload and schedule outpatient visits, ensuring productivity targets.
• Communicate with patients and families to schedule and provide ongoing support.
• Participate in rotating weekends, holidays, and shift coverage.
