Job summary

Remarkable Hospice is seeking a dedicated Registered Nurse to administer hospice care in patients' homes, skilled nursing facilities, or assisted living facilities. You will coordinate with a multidisciplinary team and assume responsibility for care coordination while working independently without on-site supervision.

Qualifications

• Education: Graduate of an accredited school of nursing.
• Experience: Two (2) years of RN experience in a clinical setting, hospice preferred.
• License / Certification: Current valid State Nursing license (RN); CPR certification; driver's license and auto liability insurance; not listed in the OIG Excluded Provider listing.

Responsibilities

• Administers medications and treatments according to policy and guidelines.
• Performs accurate assessments and develops a written Plan of Care with the patient, following physician orders.
• Documents patient status accurately and timely; communicates patient needs and interventions.
• Establishes and maintains relationships with referring physicians and patient families; coordinates care with the interdisciplinary team.
• Educates patients and families on healthcare plans, symptom control, and end-of-life care options.
• Makes visits as scheduled and participates in on-call rotation as required.
