Job summary

Logan Health Hospice is seeking an independent RN to join a compassionate interdisciplinary team providing in-home hospice care in the Flathead Valley area. You will manage your own patient caseload, travel to patients’ homes, and support symptom management, education, and comfort-focused care at the end of life.

The position emphasizes collaboration with physicians, social workers, chaplains, CNAs, LPNs, and volunteers, and requires a Montana RN license, driver’s license, and prior hospice or

Qualifications

• Current Montana RN license required or multi-state compact with Montana authorization.
• BSN degree preferred.
• Minimum 2 years’ experience in a similar medical setting preferred.
• Current Driver’s License required.

Responsibilities

• Manage own patient caseload and travel throughout Flathead Valley visiting patients at home.
• Perform in-home assessments, symptom management and comfort-focused interventions.
• Educate patients, families and caregivers on expectations and care.
• Collaborate with the interdisciplinary team to create a patient plan of care.
• Attend Interdisciplinary Team meetings and participate in on-call rotation.
• Document visits and coordinate medications, equipment, and supplies.
