Qualifications

• Montana CNA certification required.
• RQI Provider (BLS) within 7 days of hire.
• Home Health Aide certification preferred or obtained within 6 months.
• Knowledge of HIPAA and patient confidentiality.
• Strong communication, organization, and interpersonal skills.
• Ability to work independently and as part of a team.
• Basic computer skills (Microsoft Office).

Responsibilities

• Provide basic patient care under the direction of assigned leadership.
• Assist patients with activities of daily living, personal care, comfort, and mobility.
• Obtain and document vital signs and report changes to the team.
• Accurately document care provided.
• Maintain a safe, clean, and therapeutic home environment.
• Assist with supplies, equipment, and day-to-day department needs.
• Support patient privacy and confidentiality.
• Work collaboratively with the care team to prioritize patient needs.
• Support compliance with Medicare and other applicable regulatory requirements.
