Job summary

Anthem Hospice Care in Arizona seeks an experienced Registered Nurse to deliver skilled nursing in patients’ homes under physician orders and a defined plan of care. You will perform start-of-care assessments, manage medications, and update care plans while coordinating with physicians and families.

You will also supervise Home Health Aides, educate patients and caregivers, and ensure timely documentation and communication with the health care team.

Qualifications

• Currently licensed as a Registered Nurse to practice in the State of Arizona.
• Active nursing license in good standing is required for practice.
• Experience with home health care and patient teaching is preferred.

Responsibilities

• Provide skilled nursing services in patients' homes per physician orders and care plans.
• Perform start-of-care assessments and maintain medication profiles.
• Evaluate patient needs, adjust plans of care, and document outcomes.
• Notify physicians of changes and obtain new orders as needed.
• Supervise Home Health Aides and conduct supervisory visits as required.
• Educate patients and caregivers and maintain accurate health documentation.
