Compassus is seeking a Hospice Registered Nurse to provide routine and emergency assessments, educations, and evaluative services in home hospice settings. You will work within an interdisciplinary team to maintain patient comfort, manage crises, and coordinate with medical leadership to ensure high-quality care.

The role requires nursing licensure and CPR certification, with preference for hospice/home health experience. Enthusiastic collaboration and compassionate care are essential.

• Strong reading, writing, and interpretation of professional materials and regulations.
• Excellent written and verbal communication with patients, families, and colleagues.

Aufgaben

• Completes routine and emergency assessments per policy.
• Provides clinical and psycho-social services per plan of care.
• Records observations, treatments, and other pertinent information.
• Communicates with IDT, Medical Director, and attending Physician regarding symptom relief and response.
• Provides care to support patient and family in crisis and participates in case management.
• Case management of routine and emergency patient care.
• Collaborates with IDT to coordinate hospice care and ensure quality.
• Updates plan of care and communicates changes to IDT/Director of Clinical Services.
• Educates patient and caregiver on disease, care, symptom control, and wound care.
