Job summary

Talent Solutions Partners is seeking a Hospice RN Clinical Liaison / RN Hospital Liaison for Greater Manchester, NH. This full-time, Monday–Friday role covers Bedford and Manchester-area facilities, focusing on hospital referrals rather than bedside nursing.

The ideal candidate is an experienced RN with hospital med-surg background and 1+ year in healthcare sales/marketing, capable of building strong relationships with hospital teams and discharge planners. A valid driver’s license is required.

Qualifications

• Active, unencumbered RN license.
• Hospital bedside nursing background (med-surg).
• Minimum 1 year healthcare sales/marketing experience.
• Ability to build relationships with hospital clinical teams, discharge planners, case managers and physicians.
• Valid driver’s license, auto insurance and reliable transportation.

Responsibilities

• Develop and maintain relationships with hospital referral sources and healthcare professionals.
• Make regular, planned calls to secure new referrals, build census, and meet goals with referring physicians, facilities, case managers, discharge planners and social workers in hospitals and SNFs.
• Educate patients, families and clinical teams about hospice services and transitions of care.
• Support referrals, patient eligibility discussions and the admission/consent process as appropriate.
• Drive hospital referral activity and hospice census growth through service-focused outreach.
