Job summary

The Keystone Distinction Group seeks a Hospice RN Clinical Liaison to cover Greater Manchester, NH, in a full-time, Monday–Friday role (8:00 AM–4:30 PM). This hospital-focused liaison position involves building referral relationships with Bedford and Manchester-area facilities, not bedside nursing.

Ideal candidates hold an active RN license, hospital med-surg experience and some healthcare sales background. Local driving and transportation are required to support outreach and referrals.

Qualifications

• Active, unencumbered RN license.
• Hospital bedside nursing experience (med-surg).
• Minimum 1 year of healthcare sales or marketing experience.
• Ability to build relationships with hospital clinical teams, discharge planners, case managers and physicians.

Responsibilities

• Develop and maintain relationships with hospital referral sources and healthcare professionals.
• Make regular, planned calls to secure new referrals and meet goals with physicians, facilities, case managers and social workers.
• Educate patients, families and clinical teams about hospice services and transitions of care.
• Support referrals, patient eligibility discussions and the admission/consent process as appropriate.
• Drive hospital referral activity and census growth through service-focused outreach.
