Job summary

Food Management Search, Inc. is seeking a Hospice RN Clinical Liaison to cover the Greater Manchester area in New Hampshire.

This full-time, hospital-focused liaison role works Monday–Friday, 8:00 AM–4:30 PM, and involves building referral relationships with local hospital teams, discharge planners and physicians. The ideal candidate has an active RN license, med-surg experience, and at least one year in healthcare sales or marketing.

Qualifications

• Active, unencumbered RN license.
• Hospital bedside nursing background (med-surg).
• Minimum 1 year of healthcare sales or marketing experience.
• Ability to build relationships with hospital teams, discharge planners, case managers and physicians.
• Valid driver’s license, auto insurance and dependable transportation.

Responsibilities

• Develop and maintain relationships with hospital referral sources and healthcare professionals.
• Makes regular, planned calls to secure new referrals and census growth.
• Educate patients, families and clinical teams about hospice services and appropriate transitions of care.
• Support referrals, patient eligibility discussions and the admission/consent process as appropriate.
• Drive hospital referral activity and hospice census growth through consistent, service-focused outreach.
