Job summary

CareerNerds, LLC is seeking a Hospice RN Clinical Liaison / RN Hospital Liaison to serve the Greater Manchester, NH area. This full-time role focuses on hospital-focused referral development and is not a bedside nursing position.

You will build relationships with hospital teams, discharge planners, case managers and physicians, educate patients and families about hospice services, and support referrals and admission processes to grow census and ensure smooth transitions of care.

Qualifications

• Active RN license in good standing and unencumbered.
• Hospital bedside nursing background, including med-surg.
• At least 1 year of healthcare sales or marketing experience.
• Ability to build relationships with hospital clinical teams, discharge planners, case managers and physicians.
• Valid driver’s license, auto insurance and reliable transportation.

Responsibilities

• Develop and maintain relationships with hospital referral sources and healthcare professionals.
• Make regular calls to secure new referrals, grow census, and meet referral goals with physicians and hospital staff.
• Educate patients, families and clinical teams about hospice services and transitions of care.
• Support referrals, patient eligibility discussions and the admission/consent process.
• Drive hospital referral activity and census growth through service-focused outreach.
