Job summary
Key People is seeking a Hospice RN Clinical Liaison in Greater Manchester, NH. This full-time, hospital-focused nurse liaison role covers Bedford and Manchester-area facilities and is not traditional bedside nursing.
You will develop hospital referral sources, educate clinical teams about hospice services, and support admissions and eligibility discussions to drive referrals and census growth in a hospital setting.
Qualifications
- Active RN license required.
- Hospital bedside nursing experience, including med-surg.
- Minimum 1 year of healthcare sales/marketing experience.
- Ability to build relationships with hospital clinical teams, discharge planners, case managers and physicians.
- Valid driver’s license, auto insurance and reliable transportation.
Responsibilities
- Develop and maintain relationships with hospital referral sources and healthcare professionals.
- Make regular, planned calls to secure new referrals, build census, and meet goals with referring physicians, facilities, case managers, discharge planners and social workers in hospitals and SNFs.
- Educate patients, families and clinical teams about hospice services and appropriate transitions of care.
- Support referrals, patient eligibility discussions and the admission/consent process as appropriate.
- Drive hospital referral activity and hospice census growth through consistent, service-focused outreach.
