Job summary

Confidential Hospice in Greater Manchester, NH seeks an RN with hospital bedside experience to serve as a clinical liaison, covering Bedford and Manchester-area facilities. This is a hospital-focused role, not traditional bedside nursing.

You will develop relationships with hospital referral sources, educate teams and families about hospice services, and support admissions and eligibility discussions to grow census.

Qualifications

• Active, unencumbered RN license.
• Hospital bedside nursing background, including med-surg experience.
• Minimum 1 year of healthcare sales or marketing experience.
• Ability to build relationships with hospital clinical teams, discharge planners, case managers and physicians.
• Valid driver’s license, auto insurance and dependable transportation.
• Bachelor’s degree in nursing or related healthcare field preferred.

Responsibilities

• Develop and maintain relationships with hospital referral sources and healthcare professionals.
• Make regular, planned calls to secure new referrals, build census, and meet goals with physicians, facilities, case managers, discharge planners and social workers in hospitals and SNFs.
• Educate patients, families and clinical teams about hospice services and transitions of care.
• Support referrals, patient eligibility discussions and admission/consent processes as appropriate.
• Drive hospital referral activity and hospice census growth through service-focused outreach.
