Job summary

Talent With Heart in Greater Manchester, NH is seeking a clinically strong RN to serve as Hospice RN Clinical Liaison covering Bedford and Manchester-area facilities. This is hospital-focused outreach, not a bedside nursing role.

The role emphasizes building referral relationships with hospital teams, discharge planners and physicians, educating patients and families about hospice options, and driving census growth through service-focused outreach.

Qualifications

• Active RN license (unencumbered).
• Hospital bedside nursing background including med-surg.
• Minimum 1 year of healthcare sales/marketing experience.
• Ability to build relationships with hospital clinical teams, discharge planners, case managers and physicians.

Responsibilities

• Develop and maintain relationships with hospital referral sources and healthcare professionals.
• Make regular, planned calls to secure new referrals and meet goals with physicians, facilities, case managers and social workers.
• Educate patients, families and clinical teams about hospice services and transitions of care.
• Support referrals, patient eligibility discussions and the admission/consent process as appropriate.
• Drive hospital referral activity and hospice census growth through service-focused outreach.
