Job summary

Amedisys seeks a dedicated hospice RN to collaborate with the HSP Clinical Manager and Peer Mentor. You will assess patients, develop individualized plans of care, oversee lancet care changes, and coordinate with the interdisciplinary team to ensure quality, compassionate end-of-life care.

Qualifications

• Current, unencumbered RN license in the state(s) of assignment.
• CPR certification.
• Less than one year of clinical RN experience.
• Candidate must be approved by VP of Clinical.
• Familiar with communicating with people of all ages and cognitive abilities.
• Knowledge of Federal/State rules, ACHC standards, and regulatory requirements.
• Valid Driver's License and reliable transportation with company liability insurance.
• Comfort with computer use and online HR/training systems.

Responsibilities

• Provide hospice nursing and case management per patient care plans.
• Perform comprehensive patient assessments with IDT and physicians.
• Manage clinical episodes and care plan changes.
• Deliver high-quality end-of-life care and symptom management.
• Coordinate with family, caregivers, and care team; make referrals as needed.
• Initiate hospice/palliative interventions and educate patients/families.
• Supervise caseload and coordinate with PRN staff.
• Document timely and accurately at point of care.
