Job summary

AMEDISYS HOME HEALTH in Evansville, IN is seeking an experienced Registered Nurse to provide and coordinate hospice nursing services and case management aligned with patient care plans.

The RN will conduct comprehensive assessments, collaborate with the IDT, supervise care teams, ensure timely documentation, and support families while delivering compassionate end-of-life care in accordance with policy and regulatory standards.

Qualifications

• Current, unencumbered RN license for the state(s) served.
• CPR certification required.
• Less than one year of clinical RN experience.
• Demonstrated knowledge of communication with individuals of all ages.
• Familiarity with Federal/State rules and ACHC standards.

Responsibilities

• Provide and coordinate hospice nursing services and case management per patient plan of care.
• Perform comprehensive patient assessments and collaborate with IDT to develop/modify care plans.
• Oversee care plan changes and ensure quality, discharge planning, and outcomes.
• Deliver high-quality end-of-life care with symptom management and emotional support.
• Document timely and accurately at the point of care in line with industry standards.
