Job summary

Amedisys is seeking a Hospice Admissions RN to coordinate admissions for hospice patients, assess needs, and work with physicians and the interdisciplinary team to plan initial care. This on-site role serves patients in the home environment.

You will establish initial care plans, provide symptom management guidance, and perform on-call duties while maintaining documentation in accordance with policy. A valid RN license, 1 year of RN experience, and CPR certification are required.

Qualifications

• Current RN license, state(s) you are assigned to work.
• One year of clinical RN experience.
• Current CPR certification.

Responsibilities

• Coordinates admissions of hospice patients by assessing initial physical, psychosocial and spiritual needs.
• Coordinates the initial plan of care with attending physician and interdisciplinary team.
• Provides skilled interventions and instructions to patient and caregivers for symptom control and comfort.
• Acts as liaison between disciplines and facilities during admissions.
• Maintains individualized patient care progress records and submits accurate documentation.
