Job summary

Amedisys Home Health is seeking a Registered Nurse case manager to coordinate patient care in the home setting. You will perform patient assessments, develop plans of care with the team, and refer to hospice disciplines as needed while maintaining clear records and communicating with leadership.

Ideal candidates have at least one year of nursing experience, an active RN license, CPR certification, and a valid driver's license with reliable transportation and insurance.

Qualifications

• One year of nursing experience.
• Current RN license.
• Current CPR certification.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops and maintains individualized patient care progress records with accuracy and timeliness.
• Records pain and symptom management changes/outcomes as appropriate.
• Teaches patients, families and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.
