Grace Healthcare Services in New Jersey is seeking a compassionate Hospice Registered Nurse (RN) to join our Interdisciplinary Team. The RN will assess and address patient and family needs, coordinate admissions, and advocate for patient wishes throughout the care journey.

The role requires a valid New Jersey or multi-state RN license, with at least two years in acute care, and a patient-centered approach. Schedule is Mon-Fri 8:30-5:00, with travel to patient locations you visit daily across

Qualifications

• Valid New Jersey or Multi-State RN license.
• Minimum of two years of nursing experience in acute-care settings such as medical, surgical, or oncology units.
• Strong clinical practice with a focus on customer service and excellent communication abilities; ability to work independently in a fast-paced environment.

Responsibilities

• Deliver skilled nursing care as outlined in individualized care plans for terminally ill patients.
• Collaborate with physicians, social workers, chaplains, nursing assistants and other stakeholders to develop and adjust patient care plans.
• Educate and support patients' families and primary caregivers, providing guidance on care techniques and emotional support.
• Serve as a liaison between patients, families, and other healthcare providers, ensuring clear communication and advocating for patients' wishes and needs.
• Travel daily to patient locations and facilities as assigned.
