Job summary

Grace Healthcare Services is seeking a compassionate Hospice Registered Nurse (RN) to join our interdisciplinary team. You will assess and address patient and family needs, initiate interventions on admission, and maintain ongoing care plans throughout the patient journey.

Responsibilities include delivering care per plans, collaborating with the IDT, educating families, and traveling to patient sites daily. Valid NJ or multi-state RN license and 2+ years acute-care experience are required.

Qualifications

• Valid New Jersey or Multi-State RN license (RN).
• Minimum of two years of nursing experience in acute-care settings (medical, surgical, or oncology).
• Strong clinical practice with focus on customer service, communication, and independent work in fast-paced environment.

Responsibilities

• Deliver skilled nursing care per individualized care plans.
• Collaborate with physicians, social workers, chaplains, and others to develop care plans.
• Educate and support patients' families and primary caregivers.
• Serve as liaison between patients, families and healthcare providers, advocating for patient wishes.
• Travel daily to patient locations and facilities as assigned.
