Job summary

Grace Healthcare Services in Atlantic and Cape May County seeks a compassionate Hospice Registered Nurse (RN) to join our interdisciplinary team. You will assess patients, initiate appropriate interventions on admission, and maintain an ongoing, responsive plan of care for terminally ill individuals.

Key duties include delivering skilled nursing per care plans, collaborating with physicians, social workers, chaplains, and families, and advocating for patients' wishes.

Qualifications

• Active RN license in New Jersey (or Multi-State).
• Minimum of two (2) years of nursing experience in acute-care settings (medical, surgical, or oncology).
• Hospice care experience preferred and strong customer service/communication skills.

Responsibilities

• Deliver skilled nursing care per individualized care plans to ensure comfort and quality of life for terminally ill patients.
• Collaborate with physicians, social workers, chaplains, nursing assistants and others to develop and adjust care plans.
• Educate and support patients’ families and primary caregivers with care techniques and emotional guidance.
• Serve as liaison between patients, families, and other healthcare providers to advocate for patients’ wishes.
