Grace Healthcare Services in Monmouth County, New Jersey, is seeking a Hospice Registered Nurse (RN) for our Interdisciplinary Team. Schedule is Mon-Fri 8:30-5:00, with travel to patient locations as needed.

The role focuses on assessing patients, initiating interventions on admission, and maintaining a continuous plan of care for terminally ill individuals. Qualified candidates hold a NJ or multi-state RN license and have at least two years in acute care.

Qualifications

• Active RN license for NJ or multi-state
• Minimum 2 years of nursing in acute-care settings (medical, surgical, or oncology)
• Strong communication and customer-service skills; ability to work independently in a fast-paced environment

Responsibilities

• Deliver skilled nursing care per individualized care plans, ensuring comfort and quality of life for terminally ill patients.
• Collaborate with IDT members to develop and adjust patient care plans.
• Educate and support patients' families and primary caregivers with care techniques and emotional support.
• Serve as liaison between patients, families and other healthcare providers to ensure clear communication.
