Job summary

Grace Healthcare Services in New Jersey is seeking a compassionate Hospice Registered Nurse (RN) to join our interdisciplinary team. You will assess patients and families, initiate care plans on admission, and coordinate ongoing interventions to support comfort and dignity.

The role requires a NJ or multi-state RN license, at least two years in acute-care settings, and daily travel to patient locations across Middlesex, Union and Somerset counties.

Qualifications

• NJ or multi-state RN license required.
• Minimum of two years of nursing experience in acute-care settings (medical, surgical, or oncology).
• Ability to work independently in a fast-paced environment and communicate effectively.

Responsibilities

• Deliver skilled nursing care per individualized care plans for terminally ill patients.
• Collaborate with physicians, social workers, chaplains, nursing assistants, and other stakeholders to develop and adjust care plans.
