Job summary

Compassus is seeking a Hospice Registered Nurse to join our interdisciplinary team. You will perform routine and emergency assessments, provide clinical and psycho-social support, and educate patients and caregivers about care, symptoms, and the dying process.

The role includes on-call responsibilities and collaboration with the IDT to ensure high-quality, patient-centered home hospice care. Ideal candidates hold an active RN license, have 1+ year of nursing experience, and are committed to

Qualifications

• Associate or Bachelor's degree in Nursing required.
• Licensure active and unencumbered in state(s) of employment.

Responsibilities

• Completes routine and emergency assessments on each patient as indicated by policy.
• Provides clinical and psycho-social services per plan of care via case management.
• Records observations, treatments, and other pertinent information.
• Communicates with IDT, Medical Director, and attending Physician regarding symptom relief and response monitoring.
• Delivers physical and emotional interventive care to support patient and family during crises.
• Prevents unnecessary hospitalizations by supporting home-based care.
• Manages routine and emergency patient care through IDT collaboration.
• Coordinates hospice care with IDT ensuring continuity and quality.
• Updates plan of care for IDT and documents changes to RNCM or DCS.
• Assists IDT in identifying intervention needs and informs team.
