Job summary

Compassus is hiring a Hospice Admission Registered Nurse (RN) to evaluate patients for hospice eligibility and communicate care options to families. You will work with an inter-disciplinary team to develop the initial plan of care and address each patient’s needs, ensuring timely support and compassionate service.

Qualifications

• RN license in the state you work.
• Two years of experience as a registered nurse, hospice experience preferred.
• Current CPR and BCLS certification.
• Current driver’s license.

Responsibilities

• Assess patients for hospice eligibility and explain hospice care services to patients, families/caregivers.
• Collaborate with the Inter-Disciplinary Team members regarding initial plan of care, patient’s condition, and other essential information.
• Ensure all patient’s initial needs are addressed.
• Enable patients to spend quality time with their loved ones and doing the activities they enjoy.
