Job summary

Vivie is seeking a Hospice Registered Nurse (RN) to deliver compassionate home-based skilled nursing care across designated regions in Minnesota. This full-time role features 72-80 hours per pay period, Monday–Friday 8:00 a.m.–4:30 p.m., with on-call rotation every 6th holiday and occasional weekend/evening coverage.

You will provide admissions, routine visits, care conferences, supervise HHAs biweekly, coordinate with IDG, ensure accurate documentation, and uphold regulatory standards while

Qualifications

• Current RN license in Minnesota.
• CPR certification required.
• Excellent communication and emotional intelligence.
• Experience in hospice, palliative care, or end-of-life care preferred.
• Knowledge of comfort medications and pain management protocols.
• Valid driver's license and insured personal vehicle for home visits.
• Willingness to be on-call and provide emergency coverage on weekends/evenings/holidays.
• Ability to pass state background check.
• Ability to read, write, and communicate in English.

Responsibilities

• Provide clinical care and case management across admissions, routine/PRN visits, re-certifications, discharges, and end-of-life care.
• Conduct accurate assessments and complete in-home documentation; apply pain/symptom management techniques.
• Supervise HHAs at least every 14 days to ensure high-quality care.
• Collaborate with IDG members and participate in care conferences; document updates promptly.
• Uphold accountability, foster a positive team culture, and support expert care delivery.
