Job summary

Vivie is seeking a Hospice Registered Nurse (RN) to deliver compassionate in-home hospice care across Minnesota. You will conduct assessments, manage pain and symptoms, and educate patients and families while coordinating with the IDG.

Qualifications

Must hold a Minnesota RN license.
Current CPR certification.
Experience in hospice or end-of-life care preferred.
Excellent communication and empathy.

Responsibilities

Provide clinical care and case management in home hospice settings, including admissions, routine visits, recertifications, discharges, and end-of-life care.
Perform accurate assessments and document in the patient’s home, using pain and symptom management techniques.
Educate patients and families, and provide ongoing emotional support.
Supervise HHAs and offer guidance every 14 days to ensure high-quality care.
Collaborate with the IDG and participate in care conferences to discuss goals of care.
