Job summary

Vivie is seeking a part-time Hospice Registered Nurse (RN) to deliver comprehensive end-of-life care to patients in their homes across designated regions in Minnesota. You will manage admissions, routine visits, and end-of-life support, while coordinating with physicians, social workers, and chaplains to honor patient goals and comfort.

Responsibilities include supervising HHAs biweekly, documenting care plans, and participating actively in IDG meetings.

Qualifications

• Active Minnesota RN license and CPR certification.
• Experience in hospice, palliative care, or end-of-life care preferred.
• Ability to pass state mandated background check and read/write English.

Responsibilities

• Provide clinical care and case management across visit types (admissions, routine/PRN visits, recertifications, discharges, end-of-life care).
• Supervise HHAs with regular guidance every 14 days.
• Collaborate with IDG and care team; participate in care conferences and document updates.
